Release of Liability:
Recognizing the possibility of physical injury associated with sports and/or the sudden illness at an event, and in consideration for the City of North Royalton and its affiliates accepting the registrant for its baseball/softball program, I hereby release, discharge, and/or otherwise indemnify the City of North Royalton, North Royalton Baseball Boosters, and affiliated organizations, coaches, and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for baseball/software, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the baseball/softball program and/or being transported to or from the same, which transportation I hereby authorize. My son/daughter has received a physical examination by a physician and has been found capable of participating in the baseball/softball program. I hereby give my consent to have an athletic trainer, emergency personnel, and/or doctor of medicine or dentistry provide my son/daughter with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of such assistance and/or treatment.
Code of Conduct:
I hereby pledge to provide positive support, care, and encouragement for my child participating in North Royalton Baseball by following this code of ethics.
I WILL encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice or other baseball events.
I WILL place the emotional and physical well-being of all children ahead of any personal desire to win.
I WILL provide support for coaches and officials working with my child to provide a positive, enjoyable experience for all.
I WILL demand a drug, alcohol, and tobacco-free sports environment for My child and agree to assist by refraining from their use at all games, practices and baseball events.
I WILL remember that the game is for children and not for adults. I will do my best to make baseball fun for my child.
I WILL ask my child to treat other players, coaches, fans, and officials, with respect regardless of age, race, sex, creed, or ability.
I promise to help my child enjoy the baseball experience within my personal constraints by assisting with coaching, being a respectful fan, providing transportation or whatever I am capable of doing.
OHSAA / CDC Concussion Waiver:
1. By state law parents and athletes are required to receive a concussion information sheet annually for each sport.
2. Coaches, referees, or officials must remove an athlete from play if the athlete is exhibiting the signs and symptoms of a concussion during practice or a game.
These include:
• Appears dazed or stunned.
• Is confused about assignment or position.
• Forgets plays.
• Is unsure of game, score or opponent.
• Moves clumsily.
• Answers questions slowly.
• Loses consciousness (even briefly).
• Shows behavior or personality changes (irritability, sadness, nervousness, feeling more emotional).
• Can’t recall events before or after hit or fall.
• Any headache or “pressure” in head. (How badly it hurts does not matter.)
• Nausea or vomiting.
• Balance problems or dizziness.
• Double or blurry vision.
• Sensitivity to light and/or noise
• Feeling sluggish, hazy, foggy or groggy.
• Concentration or memory problems.
• Confusion
• Does not “feel right.”
• Trouble falling asleep.
• Sleeping more or less than usual.
3. The athlete cannot return to play on the same day that the player is removed.
4. The athlete is not permitted to return to play until they have been assessed and receive written clearance by a physician (MD or DO) or by any other licensed health care provider approved by the youth sports organization. PLEASE NOTE: It is important to review your organization’s policy regarding which health care providers are authorized to clear an athlete to return to play.

Lindsey Law - Sudden Cardiac Arrest:
Ohio law requires;
1. All parents and players to view the video, or attest that they did.
2. All parents and players to be given the informative pamphlet
3. The signature of players and a parent/guardian on the confirmation form.
